Workplace Injury

If an employee sustains an injury/illness in the scope of his/her employment, the supervisor should ensure that the injured employee completes the Occupational Injury/Illness Report, have it signed by the employee and the supervisor, and forward it to the Office of Risk Management and Insurance.
